Seafaring is widely recognised as a potentially hazardous occupation and, as such, seafarers need to be fit and healthy so that they can perform their roles safely. The Maritime and Coastguard Agency (MCA), who implement the government’s maritime safety policy in the UK state, “despite the development of modern technology it will never replace the most valuable resource on a ship: the crew. The requirement that seafarers must be fully fit to carry out the full range of their duties remains as important today as it did when scurvy took its toll on the first oceanic voyages”.
In addition to being fit to perform their role, seafarers should be free from any foreseeable risk of disease at sea, especially where this might spread to others, require emergency treatment, or lead to evacuation or diversion of the ship. They also need to be physically and psychologically fit to deal with emergencies at sea. Merchant Shipping Regulations make it a legal requirement for a seafarer (any person who works in any capacity onboard a commercially operated seagoing ship) to hold a medical certificate, known as an ENG1 in the UK. The MCA approve a number of doctors to undertake these medical examinations. Approved doctors assess fitness using mandatory medical and eyesight standards - currently Merchant Shipping Notice 1822 (M). A normal ENG1 certificate (for those aged 18+) lasts for 2 years.
The ENG medical involves:
- completion of a medical questionnaire
- measurement of height, weight, BMI, pulse, blood pressure and urinalysis
- detailed vision check, including colour vision
- spirometry (lung function) and audiogram
- a medical examination
- health promotion advice: to help safeguard future health and minimise risk factors where possible, such as smoking, exercise, weight control etc
- an opportunity to address any other health issues, especially those relating to work at sea
You need to bring photographic proof of ID, previous ENG certificate (unless a new appointment), and any medication, recent letters, discharge notes or sight test results which refer to treatment since your last ENG. You also need to bring any glasses or contact lenses used, as you will need to have your vision checked with and without these.
